Comments & Questions on Long Term Care

I am a licensed practical nurse in a long term care facility in South
Florida. Currently, the resident to nurse ratio is 28 to 1. In
addition, the administration tends to staff more nurses on the
weekends leaving us short during the week. How does this ratio compare
to other long term care facilities?
For me, it is barely manageable when everything goes well and there
are no new admissions, transfers or emergencies.
I have also noticed the facility's tendency to discharge residents to
the hospital who are on Medicaid, and their beds are quickly occupied
by a new resident on Medicare. Sometimes the new Medicare resident
arrives before the discharged resident has left the building!
The ADON has personally contacted me on one occasion and requested
that I find a female resident to "send out" who is not Medicare.
I have resisted this practice so far, but feel pressured to comply at
times.
Are there any nurses who have been asked to discharge a Medicaid
resident without a legitimate medical reason?

Hell Jim, that's a no brainer. Get outta there. There are lots of
places where you can get another job without that kind of crap. Suggest
that Ms. DON do the dirty work herself. Explain patiently that you are
a nurse, and your duties don't include what she requests (booting an old
person out of a facility).

That's unethical, and probably illegal, since it is fraudulant. DO
NOT become involved in that scam!! Document every time you see it
happening, times, dates, persons involved. You should also document
the details of that request by the ADON You need to protect your
license. If it were me, I would call the ombudsman for nursing homes,
and would inform the DOH and Medicaid. You can do all that
anonymously if you're worried about repercussions.
Better yet, find another place to work...where administration is
honest and upfront. Good luck.

I am an LPN in central FL who worked in local LTC. Ratio about the
same; though we didn't have quite the same "discharge/lack of care"
plan. <g>
I got fired...for spending too much time with patients and neglecting
the paperwork. Best thing that ever happened to me!!! Now work
1-on-1 in homecare through an Agency and LOVING it.
Benefits aren't quite as good, but I go home feeling good about myself
because I had time to do a thorough job.

I'd have to agree. Don't remember the exact circumstances, but the |
Psych Unit I worked on had a run of Granny Dumps where nursing homes
would all of a sudden note psychosis and unruliness in patients,
send them to us and refused to take them back, saying they were
discharged and it was our responsibility. They did that until we
went to the appropriate authority who then told them, they could
either take them back, find them a new home IMMEDIATELY or lose
their MCaire funding. Stopped that pretty much right away. I would
probably go to the local MCare people and ask them who to talk to.
