BACKGROUND: Recurrence of nasopharyngeal carcinoma after initial therapy has been reported to range between 18% and 54%. As an
alternative to surgical salvage, patients with recurrent nasopharyngeal carcinoma are offered a second course of radiation therapy.
If this second course fails, patients may be candidates for surgical resection.

OBJECTIVE: To identify the effectiveness and morbidity of surgical resection of recurrent nasopharyngeal carcinoma in patients who
have received 2 cycles of external beam radiation.

DESIGN AND SETTING: Retrospective survey of 6 patients in a university-based practice who underwent resection of recurrent
nasopharyngeal carcinoma after 2 courses of radiation therapy.

PATIENTS: Our study group comprised 4 women and 2 men aged between 35 and 67 years. All patients underwent 2 courses of radiation
with a mean total dose of 11 500 rad (115 Gy) (range, 9500-13 200 rad [95-132 Gy]) delivered to the nasopharynx prior to resection.
The mean duration between the second course of radiation and resection is 21 months (range, 8-52 months). The mean follow-up period
is 7.2 years (range, 4.2-11.5 years).

INTERVENTION: Nasopharyngectomy after failure of 2 courses of radiation therapy.

MAIN OUTCOME MEASURES: Postoperative clinical outcome and morbidity. R

ESULTS: Five years after resection, 1 patient died of disease. The remaining 5 patients (83%) are alive with no evidence of disease.
Osteomyelitis is the most common complication, affecting 5 patients. Three of the 5 patients with osteomyelitis required operative
debridement of the nasopharynx and split-thickness skin grafting. Other complications include oronasal fistula (2 patients), chronic
otitis media (2 patients), and nasopharyngeal stenosis (1 patient).

CONCLUSION: Although poor wound healing is evident, the overall 5-year survival of 83% is encouraging.

Treatment results of 1070 patients with nasopharyngeal carcinoma: an analysis of survival and failure patterns.

BACKGROUND: The aim of this analysis was to evaluate the outcomes of patients with nasopharyngeal carcinoma (NPC) treated primarily
by external beam irradiation (ERT) and to explore for possible ways to improve the treatment results.

METHODS: One thousand seventy patients with nonmetastatic NPC treated from 1990 to 1998 were retrospectively analyzed. The
distribution according to the Union Internationale Contre le Cancer (UICC) (1997 edition) staging system at initial diagnosis was as
follows: stage I, n = 113; stage IIA, n = 38; stage IIB, n = 360; stage III, n = 306; stage IVA, n = 136; stage IVB, n = 117; T1, n
= 284; T2a, n = 88; T2b, n = 398; T3, n = 149; T4, n = 151; N0, n = 321; N1, n = 393; N2, n = 238; N3a, n = 29; N3b, n = 89. Two
hundred eight patients were given neoadjuvant chemotherapy. Ninety-seven patients were diagnosed with locally persistent disease and
were salvaged with high dose rate intracavitary brachytherapy. Multivariate analysis was performed with the Cox regression
proportional hazards model.

RESULTS: The 5-year actuarial local failure-free survival, regional failure-free survival, distant metastasis-free survival,
progression-free survival, cancer-specific survival, and overall survival rates were 80.9%, 93.3%, 77.2%, 62.7%, 71.4%, and 66.5%,
respectively. Isolated distant metastasis occurred in 191 patients (18%). The distributions were as follow: stage I, 2.1% (two of
95); stage IIA, 5.7% (two of 35); stage IIB, 14.9% (45 of 302); stage III, 26.4% (62 of 235); stage IVA, 40% (40 of 100); stage IVB,
47.1% (40 of 85). Results of the multivariate analysis of various clinical endpoints were discussed.

By studying these failure patterns, it is hoped that we could refine future treatments according to the failure patterns of patients
with different risks of locoregional and distant failure.

CONCLUSIONS: The 18% incidence of isolated distant metastasis is too high to be ignored. Maximizing the local control and minimizing
the risk of distant metastasis and late complications should be the key objectives in designing future clinical trials.

Photodynamic therapy for recurrent nasopharyngeal cancer.

OBJECTIVE: A pilot study to determine if photodynamic therapy could be a safe and efficacious treatment for recurrent or
persistent nasopharyngeal cancer.
DESIGN: A consecutive sample intervention study with comparison with historic control subjects.

SETTING: Tertiary referral hospital.

PATIENTS: All patients with recurrent or persistent nasopharyngeal cancer following radiation therapy were considered for treatment.
Patients with tumors with a depth of more than 10-mm invasion on computed tomographic scans were excluded, as were patients with
recurrent metastasis to the neck. Five patients were thus acquired during a 3-year period.

INTERVENTION: Four patients were injected intravenously with hematoporphyrin derivative (2.5 mg/kg) and one patient with porfimer
sodium (2 mg/kg) (Photofrin, Quadra Logic Technologies, Vancouver, British Columbia) 48 hours before treatment. The drug was
activated by a 630-nm laser light passed down a 1-mm core quartz fiber to a miniaturized convex mirror positioned in the nasopharynx
via the contralateral nasal cavity. This procedure was carried out under topical anesthesia.

MAIN OUTCOME MEASURE: Survival was determined after a minimum follow-up of 4 years.

RESULTS: To date, three of five patients treated have no evidence of disease, with follow-up times of 51, 52, and 60 months,
respectively. The patient with the longest survival time had been unsuccessfully treated with 136 Gy of ionizing radiation preceding
photodynamic therapy.

CONCLUSIONS: Long-term tumor control can be achieved by photodynamic therapy in cases where very high doses of ionizing radiation
have failed. The entire treatment can be accomplished in 30 minutes under topical anesthesia. The technique carries no serious side
effects.
