sinus lift and bone graft

About nine months ago I had a sinus lift and bone graft done by an oral
surgeon. I had to have
this done in order to have implants on my top left side. I am supposed
to have the implants
put in sometimes this summer. After the sinus lift and bone graft I
had several follow-up
checkups and xrays. The bone graft was a success and I then had plenty
of bone for the
implants.
Well I thought everything was going along great until last week. I
started having a really
bad nasal discharge. The discharge was a yellow green color and there
were small gray
granules in it. This has been going on now for over a week. I called
my oral surgeon's
office to get an appointment to find out what was going on but he is
out of the office
for the next 10 days.
Does anyone have any imput into my situation and what are the gray
granules?

Obviously you're thinking the graft has come through the sinus
membrane, but while this seems obvious there's no way to tell. I would
save some of these granules for him to see, though.

No I don't. The membrane is pretty thin, and after the sinus lift
there is nothing separating the sinus from the mouth but the membrane,
the graft tissue, and the sutured soft tissue overlying it. I somtimes
wonder why these usually work--think of the pressures exerted on the
surgical site from a sneeze.
I'm a general dentist. I don't know that this is the graft coming out,
but obviously it must be checked. If the surgeon has been following
this and there is clear evidence that the graft has succeeded I may be
totally wrong. You may wish to check with your general dentist, who may
wish to start you on some antibiotic until you can see your surgeon, or
perhaps make a referral to an ENT specialist.

Pus and granules is not a good sign.
There is probably some infection going on and you should be on some
antibiotics ASAP or all will be lost.
Sometimes during a sinus lift there is a perforation of the membrane or
it could occur later during implant placement. It will more than likely
heal on its own but only after the infection is gone. Those particles
could be some of the graft material. The particle graft can look great
on the x-ray but it does not mean it vascularized with your regular
bone. In other words it could be just tightly packed particles that do
not stick to each other, but packed nicely to appear on the x-ray like
you have enough bone.
Oral surgeons have an emergency phone number you should contact your
surgeon to obtain a prescription for antibiotics.

Jeez... This is JUST what I want to hear, with my sinus lift and
implant surgery coming up in about 4 weeks...
Anyway, good luck!
I think if I were you I would get in to see another oral surgeon while
your regular guy is out of town. Maybe his office can refer you to
someone he respects?
Sounds like you may want to see someone asap!
